What is peyronie's disease?

By peyronie’s disease, we mean a benign abnormality of the erect penis. The abnormality in the penis is caused by hardening of connective tissue strands, resulting in the formation of 1 to three ‘plaques’ in the penis. Often, peyronie’s disease is accompanied by erectile dysfunction.

Acute inflammatory phase

The peyronie’s disease is characterised by an initial acute inflammatory phase (usually 12 months) consisting of curvature with shortening of the penis and usually painful erections. This is followed by the stable phase.

Stable phase

In the stable phase, the pain usually decreases or disappears and the curvature stabilises. An important statistic is that 9 months from the onset of symptoms in 21% of patients, the curvature increased by 10 degrees. In 12% of patients, the curvature decreased slightly. The stabilisation earlier on in older men, than in young men.

Purpose of peyronie's treatment?

ACREN Clinics’ peyronie’s treatment focuses on reducing the curvature angle, pain relief, improving the quality of sexuality experience and addressing erectile dysfunction.

Prior to peyronie treatment?

Prior to treatment, you will have an appointment with the treating urologist. Based on your photographic material, ultrasound and physical check, he will determine the size of the plaque(s).
If you also want a first treatment session right away, we will schedule it on the same day. Here, we take into account your current blood quality and your travel distance.

Important

Do you think you have the symptoms of peyronie’s disease? If so, don’t stick around too long without taking action. It is important for an experienced urologist to see if it is peyronie as soon as possible.
By walking around too long without taking action, the plaques become increasingly hard and the curvature angle may increase, requiring more treatment sessions or limiting the result of the treatment. We can only really observe the hardness during treatment.
Peyronie’s treatment consists of a minimum of 3 treatment sessions. In consultation with the doctor, it may be decided to add additional treatment sessions.

Treatment for Peyronie's disease

The peyronie’s treatment is in collaboration with you, with you ensuring proper blood quality and following the guidelines after each treatment session.

Local anaesthesia

On the day of treatment, preanaesthetic cream is applied to your entire penis. Once the preanaesthetic works, additional anaesthetic is administered by the doctor. Once the penis is fully anaesthetised, your peyronie treatment is started.

What does peyronie's treatment cost

After the examination at the consultation, you will come for treatment sessions three times every 4 – 6 weeks. The price is € 647 per treatment session. In total, at least 3 sessions are needed for your peyronie treatment, this is determined in consultation with the doctor. After that, you return annually for a maintenance treatment.
After that, you come back annually for a maintenance treatment. The price is € 647 for the maintenance treatment.
It happens that gentlemen like the effect so much that they make an appointment for maintenance after only 3-6 months. This can and may be done, whereby we rely on your own sense of touch.

Pump

If you have your own penis pump, you can use it. If you don’t have one, you will find a link to the medical device distributor’s webshop in your Personal PRP Plan. The price of the pump is about €180 (ex. shipping) in the UK.

What is needling?

Using a needle, the doctor makes fracture lines in the plaque(s), creating hinge lines and breaking the plaque(s) into small, flexible pieces.
The needling helps reduce the angle of the peyronie and minimise pain in erect penis.

What is LR-PRP?

The abbreviation PRP stands for Platelet Rich Plasma which is extracted from your blood by centrifuge. PRP 5.0 is LR-PRP where LR stands for ‘Leukocyte-Rich’. Leukocyte is the name for white blood cells. Within PRP 5.0, exosomes present in the blood are also activated.
The abbreviation PRP stands for Platelet-rich Plasma which is extracted from your blood by centrifuge. Your blood consists of ± 55% plasma, ± 44% red blood cells, <1% white blood cells and 0.2% platelets. Centrifuge and resuspension separate the plasma and platelets from the white and red blood cells.
The LR-PRP is injected into the plaque(s) to soften them. The PRP is also injected into the corpora cavernosa, shaft, corona and glans to ensure complete revitalisation of the penis.

Plasma, platelets, white blood cells and exosomes?

Plasma is 91.5% water and the remaining 8.5% consists of nutrients, vitamins, hormones, electrolytes and many blood proteins. The water in plasma makes it liquid and allows it to be injected.
Platelets are small disc-shaped cells formed in the bone marrow which contain granules containing over 300 bioactive substances and growth factors. They have no nucleus and cannot multiply. The lifespan of a platelet is 5 to 9 days. Platelets have a key role in building new connective tissue and restoring blood supply.
White blood cells provide more release of growth factors. which are directed to make blood vessels and exosomes. These are pieces of cell membrane, which ensure the activation of stem cells to make younger and new tissues or body cells.

What is pumping?

Pumping refers to the use of a medical vacuum pump to counteract the clumping of the plaque(s), after treatment, and to enhance the action of the LR-PRP.

Influencing results

You have a lot of influence on the blood quality when your blood is taken. After the injection, you have a lot of influence on the effect of your peyronie treatment with LR-PRP. How you do this we explain in your Personal PRP plan.

Personal PRP Plan

In the Personal PRP Plan, we indicate when you need to do what on which date. This is related to the three treament sessions.
For your Personal PRP plan, you will be sent a link by e-mail (no hotmail) with the login code that will take you to the page with your Personal PRP Plan.

Content Personal PRP Plan

The plan starts 4 weeks before your first treatment session and runs until 6 weeks after the third session and addresses the following points, among others:
  • Cortisone
  • Anti-inflammatories
  • Antibiotics
  • Blood thinners
  • Cholesterol
  • Warmth
  • Exercise
  • Alcohol, tobacco, caffeine and drugs
  • Pump operating instructions
An additional effect of LR-PRP is an increase in penis volume and sensitivity.
You can observe change from: 6 weeks after the first treatment session to 6 months after the last session.

Blood thinners

Peyronie’s treatment is also possible if you take blood thinners. The condition is that your INR (blood clotting value) is between 1 and 2 on the day of the treatments.

Your peyronie treatment

In advance

  • You have clicked on the MAKE AN APPOINTMENT button, filled in the request form and pressed submit.
    The request arrives at our service desk.
  • The medical assistant who will accompany you calls you to schedule an appointment with the urologist .
  • After the interview with the urologist, you will be called again by the medical assistant to set the dates for the first three treatment sessions.
  • After the three treatment dates and times have been fixed, the medical assistant will send you an e-mail message containing the appointment confirmation, the address of the private clinic, the name of the doctor, the link to your Personal PRP Plan and password.
  • The medical assistant will ask you to send a reply once you have received the e-mail so that you can start preparing.

Treatment session

  • The nurse will welcome you and take you to the treatment room. Here you can lie down on the treatment table after removing your underwear.
  • The nurse will take two tubes of blood and put them in the centrifuge.
  • Then the nurse smears pre-anaesthetic cream all over your penis. This gives a warm feeling.
  • After 5 minutes, the centrifuge is finished and you feel that the preanaesthetic is working.
  • The nurse prepares your LR-PRP for injection.
  • In the meantime, the doctor will come and give two numbing injections at the base of your penis, making it completely numb. The more you relax, the less you will feel from the anaesthetic.
  • Once the anaesthetic is working, the doctor will start the needling and then inject the PRP on a quiet one.
  • After that, you are ready to get dressed.
    The treatment session lasts 30 – 40 minutes.

Afterwards

  • You follow the instructions you received through your Personal PRP Plan.

Follow-up

  • Three months after your last treatment session, your medical assistant will contact you to discuss your progress. In consultation with you, the next contact will be arranged.
Making love in the period between treatment sessions is certainly possible if you feel it can be done.

Do you use ESWT in the Peyronie treatment?

No, we deliberately chose to apply needling to create breaks in the plaque(s) and stimulate blood vessels in combination with body’s own material in the form of LR-PRP and exosomes.

What to do about Peyronie's disease?

Currently, there are no drugs that can cure Peyronie’s disease. Surgery can straighten your penis, but has other adverse effects. If you would like surgery, it can be done once the disease has stabilised. At ACREN Clinics, we opt for the combination of Needling, LR-PRP (PRP 5.0), Personal PRP Plan and pumping. We cannot cure the disease. Treatment focuses on addressing the discomforts of Peyronie’s disease.

How does Peyronie look like?

In Peyronie’s disease, your penis curves when getting an erection. A slight curvature that many men experience when curving is normal. Sometimes the curvature is painful in erect position of your penis. The curvature is not necessarily upward. You may feel a hardening when your penis is ‘flaccid’. It is recommended to have Peyronie’s diagnosed by an experienced urologist.

Is Peyronie dangerous?

The hard spot(s) that plaque(s) in your penis are not dangerous. This is called plaque or plaques if multiple hard spots can be observed.

How many people have Peyronie's disease?

It is a rough estimate that 1 in 15 men, usually over 40, will be affected by Peyronie’s disease.

What can be mistaken as Peyronie?

Peyronie’s disease is sometimes confused with congenital curvature or penile fracture. It is common to have penile cancer, scelordermia (a form of inflammation or hardening of connective tissue) or other another form of fibrosis (connective tissue formation). It is important to have a Peyronie’s diagnosed by an experienced urologist.

Can Peyronie disappear?

Complete disappearance of Peyronie’s disease has not yet been observed. A slight improvement may occur sponistically after stabilisation of the disease. Often, pain then also decreases. This is about a year from the first symptoms.

Can you live with Peyronie?

If you are sexually active and the curvature makes penetration impossible or painful, then you can think about treating your Peyronie’s. If you have no pain and it does not bother you, then you can live with Peyronie’s without starting treatment.

Can you have sex with Peyronie's disease?

Whether this is painful depends on the Peyronie’s disease and its stage. The angle of the curvature may make it difficult or even impossible to penetrate. If, as a man, you do not undergo symptoms of pain, there is no reason not to have sex.

Waarom geen Nesbit procedure?

The Nesbit procedure helps surgically correct the curvature of the penis. Our surgeons have mastered the technique, it has too many negative effects including shortening of the penis and the plaques are not broken which can lead to worsening of the discomfort before surgery.

Why no Xiapex?

Xiapex is no longer available in Europe. Something would exist via England and then to Rome, but have been unable to get in touch with the organisation themselves. Xiapex’s results were limited, beyond the fact that it was an expensive treatment.

Does ACREN Clinics use ESWT?

ACREN Clinics has replaced the extracorporeal magnetic transoduction (ESWT), Xiapex, Nano VI and Nesbit procedure with the combination of needling, PRP 5.0, pumping and Personal PRP plan.
